Volleyball KC

Participant confirms they are 18 years or older and voluntarily participating in Volleyball KC outdoor grass 6v6 coed volleyball activities, including leagues, games,
practices, tournaments, and related events.

Participant understands participation involves inherent risks including physical exertion, collisions, weather conditions, uneven surfaces, equipment hazards, and
negligence of other participants. Participant voluntarily assumes all known and unknown risks.
Participant agrees to inspect fields, courts, equipment, and surrounding conditions prior to play and accepts all conditions “as-is.” Participant agrees to follow league
rules and behave safely and respectfully. Volleyball KC may remove participants for unsafe or inappropriate conduct.

To the fullest extent permitted by Missouri law, Participant releases and discharges Volleyball KC and its owners, employees, volunteers, sponsors, affiliates,
landowners, and agents from claims arising out of participation, including those caused by ordinary negligence. This release excludes gross negligence or willful
misconduct where prohibited by law.

Participant agrees to indemnify and hold harmless Volleyball KC from claims arising from participation or conduct, authorizes emergency medical treatment if
necessary, accepts responsibility for medical expenses, understands no insurance is provided, and grants permission for photo/video use. Missouri law governs this
Agreement.

By signing below, Participant acknowledges they have read, understood, and voluntarily agree to this waiver and release of liability.
